Key Responsibilities

The Practice Nurse will provide a broad range of primary care nursing services within their scope of practice.

Responsibilities include:

• Following up patient test results and communicating with patients where appropriate
• Carrying out clinical interventions and treatments
• Conducting NHS health checks and health screening
• Supporting asthma and diabetes reviews
• Providing cervical screening services
• Delivering immunisation programmes
• Providing health education and lifestyle advice
• Supporting contraception and sexual health services
• Managing patients with chronic conditions
• Supporting people with long-term and terminal conditions
• Contributing to health promotion and public health programmes
• Coordinating patient care and referrals
• Contributing to quality assurance, audits, and practice improvement

Experience or an interest in chronic disease management, minor illness, sexual health, health promotion, and mental health would be particularly beneficial.
