  • Assess and clinically triage referrals.
  • Identify urgent and non-urgent patient needs.
  • Coordinate same-day and routine community visits.
  • Provide clinical advice when a face-to-face assessment is not required.
  • Communicate with District Nursing teams and other community services.
  • Prioritise patients safely according to clinical need.
  • Contribute to the development of the triage service.

Strong communication and decision-making are particularly important because the nurse needs to gather relevant information and determine the appropriate next step.

What Qualifications and Experience Are Required?

This vacancy is open to nurses who meet the required professional registration and educational criteria.

Applicants must have:

✔ Current UK professional registration as an adult/general nurse

✔ Degree-level education or equivalent

✔ Knowledge of long-term conditions and their management

✔ Ability to work under pressure and manage competing priorities

✔ Basic IT and keyboard skills

✔ Understanding of clinical governance

Previous community nursing experience is listed as desirable rather than essential. Experience in areas such as wound care, medication administration, and catheterisation is also desirable.
