Job Overview
Midlands Partnership NHS Foundation Trust (MPFT) is recruiting a Band 5 Triage Nurse – Community Nursing to join its North Alliance Locality Access Point (LAP) Service. This role is ideal for NMC-registered nurses with community nursing experience who enjoy clinical decision-making and remote patient assessment.
The position primarily involves conducting telephone-based clinical triage, reviewing referrals, prioritising patient care, and ensuring patients receive the right treatment at the right time. Although the role is mainly remote after training, nurses will occasionally provide community visits to maintain clinical competence.
Important: This position does not provide visa sponsorship. Applicants requiring Skilled Worker visa sponsorship are not eligible.
Key Responsibilities
As a Triage Nurse, your responsibilities include:
- Conduct comprehensive remote clinical assessments.
- Prioritise referrals according to patient needs.
- Allocate urgent community nursing visits.
- Provide telephone advice for patients, carers and families.
- Assess eligibility for community nursing services.
- Redirect inappropriate referrals to suitable healthcare services.
- Liaise with community nursing teams and multidisciplinary professionals.
- Support self-management where clinically appropriate.
- Recognise deteriorating patients requiring urgent intervention.

Working Pattern
Initial training is provided at:
Weaver House, Cheadle
Following training, the role combines:
- Office working
- Home working
- Occasional community visits
Shift patterns include:
- 7:00 am – 3:00 pm
- 9:00 am – 5:00 pm
- 10:00 am – 6:00 pm
Service operates 7 days per week.
