A strong OSCE clinical examination is systematic, safe, and patient-centred. Examiners assess not only whether you perform the right steps, but also how you explain them and respond to the patient.
Use a consistent examination routine
Before beginning, introduce yourself, confirm the patient’s identity, explain the examination, and ask for consent. Check whether the patient is comfortable and whether they have any pain that could affect the examination.
Follow a logical sequence for the system you are examining. Practise the same order each time so you are less likely to miss a step under pressure. Maintain the patient’s dignity by exposing only what is necessary, offering a chaperone when appropriate, and protecting their privacy.
As you examine, explain what you are doing in plain language. Give clear instructions when you need the patient to move or change position, and check that they are comfortable. When finished, thank them, help them get dressed or settled, and explain what you would do next.
Make communication part of the examination
Good communication supports the whole encounter. Listen to the patient’s concerns, avoid jargon, and respond calmly if they seem anxious or uncomfortable.
Practise speaking while you perform the examination. Ask a partner to observe whether your instructions were clear, whether you checked consent, and whether you treated the patient respectfully.
Practise, review, repeat
After each practice station, identify one strength and one specific improvement. Then repeat the station, focusing on that improvement. Regular timed practice can help you build a routine that feels both efficient and considerate.69
In an OSCE, a careful examination and clear communication work together. A systematic approach helps you demonstrate your clinical skills while helping the patient feel informed and respected.
