  • Assessing incoming patient referrals.
  • Identifying urgent, same-day, and routine care requirements.
  • Coordinating visits by District Nursing teams.

  • Signposting patients to alternative services when suitable.
  • Working with community and wider healthcare services.
  • Supporting the development of the District Nurse Triage service.

This makes strong clinical assessment, communication, prioritisation, and decision-making skills particularly important.

Working Pattern and Location

The service operates seven days a week, 365 days a year, with core operating hours from 8:00 am to 6:30 pm.

Available shift patterns include:

  • 8:00 am – 4:00 pm
  • 10:30 am – 6:30 pm
  • 8:00 am – 6:30 pm

This is primarily an office-based triage position, and the successful candidate is required to work on site.
