About the Triage Nurse Position
The successful nurse will join the District Nurse Triage Team, which assesses referrals received through telephone and email.

- Assessing incoming patient referrals.
- Identifying urgent, same-day, and routine care requirements.
- Coordinating visits by District Nursing teams.

- Signposting patients to alternative services when suitable.
- Working with community and wider healthcare services.
- Supporting the development of the District Nurse Triage service.
This makes strong clinical assessment, communication, prioritisation, and decision-making skills particularly important.
Working Pattern and Location
The service operates seven days a week, 365 days a year, with core operating hours from 8:00 am to 6:30 pm.
Available shift patterns include:
- 8:00 am – 4:00 pm
- 10:30 am – 6:30 pm
- 8:00 am – 6:30 pm
This is primarily an office-based triage position, and the successful candidate is required to work on site.
