Can SIFE replace OET/IELTS for UK NMC?

For internationally educated nurses aiming to practice in the UK, navigating the English language requirements is a critical step in the registration process. A common question that arises is whether Supporting Information from Employers (SIFE) can completely replace the standard OET or IELTS examinations.

What is the SIFE Pathway?

Introduced by the Nursing and Midwifery Council (NMC), SIFE (Supporting Information from Employers) allows UK employers to provide an official reference detailing an applicant’s real-world English competence observed in a clinical setting. It was designed to help qualified overseas nurses who are already working in the UK but are held back by language test hurdles.

However, you cannot simply choose SIFE as a direct substitute for an English test from your home country. You must fulfill specific eligibility gates to use this pathway.

Who is Eligible to Use SIFE?

To utilize supporting employer information instead of standard OET/IELTS test scores, you must satisfy both the employment criteria and one of the language criteria paths outlined below:

1. The Strict UK Employment Criteria

  • Duration: You must have worked in a non-registered healthcare role (such as a Healthcare Assistant or Support Worker) in a UK health or social care setting for a minimum of 12 months full-time (or part-time equivalent) at the point of application.
  • Employer Continuity: You must have been with the same UK employer for those 12 months.
  • Management Sign-off: You must have had the same line manager (who must be an active NMC registrant) for at least 6 months. The reference must also be countersigned by a senior manager who is an NMC registrant at NHS Band 8a (or non-NHS equivalent) or above.

2. The Language Evidence Gates (Must meet one)

  • Route A (Trained in English): You completed your pre-registration nursing program in a country where English is not the majority spoken language, but your specific program was fully taught and assessed in English. SIFE acts as evidence of your actual clinical interaction skills.
  • Route B (Narrow Test Miss): You have attempted at least two formal English language tests (either IELTS or OET) and narrowly missed the required score in just one single domain by no more than 0.5 in IELTS or half a grade in OET, having already exhausted all standard test-combining options.

When OET or IELTS is Absolutely Required

If you are applying directly from an overseas country (like India) and have not yet lived or worked in a non-registered healthcare role in the UK for a full year, SIFE is not an option for you.

You will be required to take the standard pathways recognized by the NMC:

  • OET (Occupational English Test): Achieving a minimum of Grade B in Listening, Reading, and Speaking, and a C+ in Writing.
  • IELTS Academic: Achieving a minimum score of 7.0 in Listening, Reading, and Speaking, and a 6.5 in Writing.
